*· To be considered an active established patient, you should be seen at least annually.
* Appointments should be requested via the patient portal whenever possible. If the patient portal is unavailable, please phone our office and select option 2 from the greeting.
* We reserve a number of appointments daily for sick calls that are filled on a first come first served basis. Please request your same day sick appointment via the patient portal and we will do our best to accommodate your needs.
* Please bring all of your medications in their original bottles to each appointment. This ensures that we have the most current information available during your visit.
* Prescription refills should be requested via the patient portal whenever possible. If unable to access portal, please call and select option 3 from the greeting. Leave a detailed message and your prescription will be renewed by the end of the next business day. Requests received over the weekend will be renewed by the end of business on Tuesday.
* Due to the recent implementation of the NY State Prescription Monitoring Program, requests for refill of controlled substances must be received 48 hours in advance of their due date. The prescription will be processed and available for pickup after 2:00 on the day they are due for refill/renewal.
